If the lord of that sign is situated in Badhaka place or is the lord of a Badhaka house, then only it causes problems.

Cause of greater suffering than that caused by Saturn, an enhancer of distress and losses, this sub-planet becomes even more malefic, if conjoined with or aspected by Saturn. If Mandi is placed in the 8th house, the native dies at a young age. Even when several planetary positions, giving longevity, are there in the birth chart but Mandi falls in the 8th house, the person has a short life span.
